Information and Consent Form

Thank you for participating in this study, Student Involvement in the Clubs of the Department of Agricultural Economics. The purpose of this study is to identify factors affecting students' involvement in departmental club activities. This study will involve 560 Internet users, who are over eighteen years of age. The survey will take 10 minutes to complete. Do not add your name or other identifying data to the survey. Please note the following characteristics of this study:
  • your participation is voluntary;
  • your identity will remain anonymous;
  • the IP address of your computer will not be recorded;
  • you can elect to withdraw at any time without penalty;
  • there are no positive or negative benefits from responding to this survey;
  • there is no compensation;
  • the survey will be used for research;
  • the results will be printed and kept for 12 months in a locked file and then destroyed;
  • the data obtained from this survey may be published.
